1. Personal Information

Identification

  •  Government-issued ID (Driver’s License, Passport, etc.)
  •  Social Security Cards (for yourself, spouse, and dependents)
  •  ITIN (if applicable)

Previous Tax Return

  •  Last year’s federal and state tax returns (if applicable)

Bank Information

  •  Bank account & routing number (for direct deposit of refunds)

2. Income Documents

Employment Income

  •  W-2 Forms (Wage and Tax Statement) from all employers

Self-Employment Income & Business Owners

  •  1099-NEC / 1099-MISC (Non-Employee Compensation)
  •  Business income & expense records (Profit & Loss Statement)
  •  Business asset purchases/sales (for depreciation purposes)

Investment & Stock Income

  •  1099-B (Stocks, Bonds, Options, Cryptocurrency sales, RSUs)
  •  1099-DIV (Dividends received from investments)
  •  1099-INT (Interest Earned from banks or investments)
  •  Stock Options / RSUs (Restricted Stock Units) & ESPP (Employee Stock Purchase Plan):
    •  Employer stock grants and vesting statements
    •  Stock sales, exercise records, and cost basis documents
    •  Employer-provided RSU tax withholding details

Cryptocurrency Transactions

  •  1099-B / 1099-K (if received from exchanges like Coinbase, Binance, Kraken, etc.)
  •  Crypto trading records (purchase price, sale price, dates, and transaction fees)
  •  NFT transactions & staking income (if applicable)
  •  Crypto mining & rewards income (earnings from staking, mining, or DeFi yield farming)

Rental Property Income

  •  Rental income and expense records
  •  Mortgage interest statements (Form 1098)
  •  Property tax & homeowners insurance statements
  •  Depreciation records (for tax deductions)

Retirement & Social Security Income

  •  1099-R (Retirement distributions from IRAs, 401(k), pensions)
  •  SSA-1099 (Social Security benefits received)

Other Income

  •  1099-G (Unemployment benefits, state tax refunds)
  •  Alimony received (if applicable)
  •  Gambling/Lottery winnings (W-2G)
  •  Jury duty pay
